Moroz M. Functional state of the myocardium in post-infarction aneurysms of the left ventricle

The dissertation is devoted to the problem of functional changes of the myocardium in post-infarction aneurysm of left ventricile (LV). In this paper, an analysis of the results of a survey of 160 patients with postinfarction left ventricile aneurysms is presented. The problem is presented as the first domestic study in which new criteria for myocardial functional changes in postinfarction LV aneurysms were justified and used: longitudinal deformation, rotation and twisting. The results of the conducted studies made it possible to determine the level of changes in these criteria depending on the lesion of the coronary arteries, the localization of the aneurysm, the type of motion of the non-aneurysmal part of the LV. The use of longitudinal deformation parameters made it possible to optimize the diagnosis of aneurysms (delineating the zone of resection or giving grounds for abandoning resection when only LV segmental septa was affected), helping in choosing the procedure of surgery (linear plastic – in the defeat of the anteroposterior segments, endoventriculoplastyc – in the spread of the lesion to IVS or on the posterior wall or lateral surface beyond the base of the anterior group of papillary muscles).
