The thesis is devoted to the development of prediction technique of the formation of pathological postoperative facial scars and to the study of the effectiveness of their prevention through the combined use of extracorporeal shock wave therapy (ESWT) and silicone gel. According to the results of an experimental study in 16 male rabbits, the combined use of ESWT and silicone gel incorporates the positive qualities of both methods and promotes the formation of the normotrophic scar with balanced processes of epidermis formation and regeneration of collagen and elastic fibers along with dermal appendages. Based on the examination of 50 patients who underwent the suggested preventive measures, the cosmetic characteristics of postoperative skin scarring improved regardless of the type of wound healing. Stitch tracks from healing by primary intention disappeared in 86,67±8,78 % of patients with the postoperative wounds. Consistency, pigmentation, vascularization,and disappearance of surgical stitch tracksimprovedonthe application. Hyperechoic (formed) postoperative scars were formed, homogeneous in structure, with clear smooth contours, without signs of edema or inflammatory infiltration. This is achieved by combining the use of shock waves, which improve blood flow at the capillary level, stimulate metabolic and reparative processes in tissues, and silicone gel, which promotes the hydration of postoperative scar. The combined effect of both components creates conditions for the proportional formation of collagen and elastic fibers reducing the size of the postoperative scar compared to the initial state.
