Essay

The dissertation is devoted to increase of efficiency of treatment of patients with periodontal diseases by experimental substantiation of use of nanoparticles of silver and gold for treatment of periodontal diseases and development of possible forms of medical preparations containing nanoparticles of these metals. The results of microbiological investigations have shown that practically all samples with Au and Ag content have a good antimicrobial activity, both on the reference test strains of microorganisms, and in the mixed microflora of periodontal pockets, to a greater or lesser extent. A comparative study of antibacterial activity of gel with nanoparticles Au and conventional antibacterial drugs did not reveal a significant difference between them. Experimental studies showed that no pathological changes, deviations from the normal physiological state of animals and the oral mucosa occurred during the application of the drug to the skin and when administered into the stomach. No changes in hematological parameters and morphological changes in the internal organs of animals were detected. The conducted complex experimental study showed that the gel "Nanozoloto" does not cause toxic and sensitizing effects on the organism and the oral mucosa of experimental animals. In the third stage of experimental studies, the effect of the mucosal gel on the periodontal tissue of 35 experimental animals was determined in the case of modeling of the experimental periodontitis model. For the treatment of animals, the periodontal tissue was applied to the mucosal gel with an addition of silica gel with gold nanoparticles. In the homogenates of the gums and the alveolar bone was determined the activity of a number of biochemical indices. Thus, in the case of experimental periodontitis, the use of mucosal gel "Nanozoloto" reduces the degree of microbial contamination, increases the level of non-specific immunity, significantly reduces the degree of atrophy of the alveolar bone, increases the content of hyaluronic acid in the gums. Experimental studies of mucosal gel "Nanozoloto" showed its pronounced therapeutic effect in experimental periodontitis due to its antibacterial, anti-dysbiotic, anti-inflammatory and antioxidant properties. Clinical and laboratory examination of 79 patients with the I degree of generalized periodontitis, chronic and exacerbated course showed a clear therapeutic effect of the use of mucosal gel "Nanozoloto". This is confirmed by clinical and biochemical studies of patients in the long term of observation: 6 and 12 months. The conducted clinical-laboratory and biochemical studies in the long term of observation showed the expressed therapeutic efficacy of the use of mucosal gel with gold nanoparticles in the complex treatment of patients with generalized periodontitis.
