The dissertation is devoted to the optimization of the prediction of clinical course and treatment of patients with coronary heart disease in combination with type 2 diabetes mellitus based on the study of P-selectin and Galectin-3. It has been established that levels of P-selectin and Galeсtin-3 in patients with stable angina pectoris show a relationship between them, but vary significantly at the individual level, with both biomarkers of inflammation and not related to the level of hs-CRP, which creates preconditions for the personalization of therapeutic goals to reduce the systemic inflammatory response. Level of P-selectin and Galeсtin-3 was influenced by the sex of the patients with angina, age of patients and the age at which angina was first detected. At the same time, various mechanisms of inflammatory response prevailed in different age groups. However, the greatest influence on the level of P-selectin and Gautlekina-3 had the degree of atherosclerotic damage to the coronary arteries and the severity of angina according to the loading tests, and especially, the history of revascularization was postponed. In the cumulative analysis of events in patients divided into tertils depending on the initial level of P-selectin, it was found that in patients with group 1 (top three on the level of Р-selectin) the number of events was significantly lower than in patients in group 3 ( lower third on the original level of P-selectin.In patients treated with clopidogrel in a dose of 75 mg / d, the level of P-selectin was significantly lower compared with patients who did not receive antiplatelet therapy and patients who received monotherapy with aspirin.
