Essay

The thesis is on the role of systemic inflammatory process in ankylosing spondylitis and arterial hypertension, as related with impaired endothelial function, course of disease, structural and functional changes in the cardiovascular system. It was founded that in patients with comorbid pathology present higher activity and the onset of the disease was noted at a younger age. Patients with arterial hypertension had higher indices of VAS, ASDAS, BASDAI, and BASFI with significantly higher levels of CRP and ESR. Multiple regression analysis, making possible to foresee endothelium vasodilation capacity with no invasive technique applied, has been performed. Mathematical model of development of arterial hypertension in patients with ankylosing spondylitis was developed. Ten-day application of L-arginine in the integrated management of patients with ankylosing spondylitis, followed by one-month oral solution intake, has been found to provide essential decline in endothelial dysfunction manifestations. Combined quinapril and L-arginine application revealed improved endothelium vasoregulatory function and arterial pressure control in patients with arterial hypertension.
