Essay

The target of research is the course and pathogenesis of ankylosing spondylitis (AS), which onset in childhood and adulthood; the goal is to improve the quality of the diagnostics, to establish new links in pathogenesis and to distinguish prognostic criteria of the AS course, which onset in childhood and adulthood, methods – clinical, X-ray, ultrasound, immunobiochemical, immunoenzyme, statistical; novelty and results – the clinical and pathogenetic differences of the AS, which onset in childhood and adulthood are first proved, in particular, the nature of radiological and sonographic signs of the peripheral articular syndrome, the degree of its severity and progression, spinal cord injury, sacroiliac joints, enthesis, choroids, central and peripheral nervous system; the peculiarities of pathogenetic significance of antibodies to cyclic citrullinated peptides, C-reactive protein, fibrinogen, immunoglobulins A, G, M, circulating immune complexes, interleukine-1β and tumor necrosis factor -α in case of juvenile and adult AS are first revealed; the criteria, which allow to characterize the severity of certain signs of the disease and to predict the course of AS in patients with different age debut process are substantiated; it is implemented in the medical process of the Clinical Facility “Primary Health Care Center No. 1 in Kramatorsk”, Municipal Medical Facility “Children's Territorial Medical Association of Kramatorsk”, the Rheumatological Departments of the Research Institute of the Disabled Rehabilitation (educational and scientific medical complex) at the Vinnytsia National Medical University named after M.I.Pirogov, Medical Facility “Dniprovska City Clinical Hospital No. 11”, Lviv Regional Clinical Hospital, Center for Rheumatology at the “Theophiania” Clinical Hospital of the State General Affairs Department (Kyiv), as well as into the pedagogical process of the Donetsk National Medical University; the sphere of application – the medical-diagnostic process of rheumatological, therapeutic, pediatric departments, the educational process of therapeutic and pediatric profiles departments at medical educational institutions.
