Essay

Object of the research are risk factors for fractures of the lower limb bones (FLLB), indicators of the lower extremity functional activity, mineral density of bone tissue (MDBT) and vitamin D level; the goal is to increase the effectiveness of FLLB prophylaxis on the basis of studying their epidemiology, risk factors and the effect on functional recovery in patients of all ages; methods – epidemiological, clinical, questionnaires, instrumental (orthopedic and anthropometric examination, two-photon X-ray absorptiometry), laboratory, mathematical-statistical; novelty and results – age and sexual characteristics of frequency of FLLB in general and fractures of the femur, its proximal department, fractures of the tibia and the foot in particular in the population of Vinnytsia district of Vinnytsia region were studied, as well as their share in the general structure of fractures depending on age and sex of the population; FLLB risk factors in patients of different ages and sexes were detected depending on the localization of the fracture; MDBT and 10-year probability of major osteoporotic fractures and femoral fractures were determined using Ukrainian FRAX questionnaire model (Fracture Risk Assessment Tool) in patients with different localization of FLLBs; level of vitamin D in blood serum of aged groups patients with fractures of the shin bones is determined depending on the gender and the proportion of its deficiency is determined; indicators of functional activity of the lower extremity in patients with fracture of the femur and bones of the leg are determined depending on age, sex, body mass index, fracture localization, serum vitamin D levels and duration after fracture; The results of the epidemiological study conducted within the framework of the Ukrainian multi-center epidemiological study «STOP» (SysTem of OsteoPorotic Fractures Registration) resulted in the creation of a Ukrainian version of the FRAX questionnaire on the 10-year risk of major osteoporotic and femoral fractures; the opportunities of using LEFS (Lower Extreme Functional Scale) questionnaire to monitor functional recovery of patients with different FLLBs and the association of its parameters with risk factors for fractures were studied; the research results have been introduced - at the department of age-related changes of the musculoskeletal system of State Institution "D. F. Chebotarev Institute of Gerontology NAMS of Ukraine", at the Department of Traumatology and Orthopedics of Vinnytsia National Pirogov Memorial Medical University of Ministry of Health of Ukraine, Department of Traumatology and Orthopedics with Military Field Surgery of Ternopil State Medical University named after I. Ya. Gorbachevsky of Ministry of Health of Ukraine, at educational-scientific-medical complex of Research Institute of Rehabilitation of Disabled People of Vinnytsia National Medical Center of VNMU, Ministry of Health of Ukraine, 4th City Communal Clinical Hospital in Lviv; the field of applying is medical and diagnostic process of specialized orthopedic and traumatological departments, outpatient-polyclinic health care units, scientific and pedagogical processes of the departments of traumatology and orthopedics, general practice – family medicine of medical educational establishments.
