Essay

The work is devoted to the improvement of the treatment results of patients with talus fractures by improving diagnostics and a differentiated approach to surgical treatment. The sensitivity, accuracy and specificity of the instrumental methods used for the diagnosis talus fractures (X-ray in standard and additional projections, CT, MRI) have been studied and calculated. The anatomical parameters and biomechanical characteristics talus fractures are specified, using the new design of the countercompressing screw for metallosteosynthesis talus fractures, and during the field stand experiment the reliability, stiffness, strength and durability of different systems of fixation of fractures talus fractures, on the basis of the biomechanical substantiates most efficient way of introduction fixing screws with different types of fracture osteosynthesis talus fractures. Developed and implemented an algorithm for diagnosis and treatment of patients with talus fractures. The causes of complications and unsatisfactory results of treatment were systematized, additional ways of avoiding them were identified in patients of the chosen category. The application of the diagnostic and treatment algorithm in patients of the main group made it possible to get the best functional results (AOFAS) and restore the foot function almost to the level of norm in 76% of patients, which is 1.9 times more compared with patients of the control group - 40%.
