Essay

The dissertation is devoted to the optimization of treatment of dyslipidemia in 161 patients with rheumatoid arthritis and Bcl1 polymorphism of the glucocorticoid receptor gene with risk factors for cardiovascular diseases in patients of this category. It has been proved that the G/G genotype is associated with the development of rheumatoid arthritis, coronary heart disease, arterial hypertension, obesity and seropositivity of rheumatoid arthritis by the content of antibodies to the cyclic citrulline peptide. The G/G genotype carriers, along with seropositivity, the disease duration of more than 10 years and the presence of visceral lesions, have a higher content of low density lipoprotein cholesterol and a lower response to atorvastatin therapy. Determination of the genotypes for the Bcl1 polymorphism of the glucocorticoid receptor gene allows to individualize the appointment of atorvastatin in patients with rheumatoid arthritis depending on the risk of cardiovascular mortality by the SCORE scale in order to optimize dyslipidemia treatment. Key words: rheumatoid arthritis, ischemic heart disease, arterial hypertension, dyslipidemia, Bcl1 polymorphism of the glucocorticoid receptor gene, SCORE, atorvastatin. Branch-Medicine.
