Essay

The dissertation is devoted to increase the efficiency of early diagnosis and prevention of anthracycline cardiotoxicity in patients with acute leukemia taking into account concomitant ischemic heart disease (IHD). The study involved 147 patients with newly diagnosed acute leukemia, of which 66 (44.9 %) patients had concomitant IHD. The lipid peroxidation processes potentiation, antioxidant defense reduction and development of endothelial dysfunction is observed at low cumulative doses of anthracyclines. It is shown that Holter ECG monitoring allows to diagnose myocardial “silent ischemia” as one of the main anthracycline cardiotoxicity symptoms in time. The appointment of L-arginine reduces the risk of developing myocardial injury in patients with acute leukemia and concomitant IHD during induction chemotherapy.
