Viun G. Optimization of methods of orthopedic treatment of periodontal disease with fixed splints

The dissertation is devoted to increase the efficiency of the complex treatment and the optimization of methods of orthopedic treatment of periodontal diseases by means of non-removable splints. The aim of the dissertation thesis is improvement of the methods of orthopedic treatment of periodontal tissue diseases with the use of non-removable splint of the author's own design. The stress of the dental-alveolar complex has been analyzed at different degrees of alveolar bone loss and in the stabilization of teeth by various types of splints constructions. It has been established that inclusion in the calculations of the model of universal folding periodontal splint allows to distribute stress more physiologically and uniformly, and significantly reduce stresses in the dental alveolar complex. According to the results of the study, application of the developed algorithm for prediction of the results of treatment of periodontitis, as well as the computer program created on its basis, which allows to predict long-term results of the treatment with high accuracy to its beginning, which helps to optimize the treatment of diseases of periodontal tissues. According to the results of the clinical and radigraphic examinations, the conservative treatment following the above method leads to a significant improvement in the state of periodontal tissues, and application for stabilizing the mobile teeth of the splint: universal folding periodontal splint - allows stabilizing the pathological process and maintaining the results in the stage of stable remission. Clinically verified results of the application of the developed universal folding periodontal splint testify to the promotion of stabilization of the pathological process in the long-term observation period (12 months). The improvement of the oral hygienic status in the main subgroups, with the use of the universal folding periodontal splint, allowed to maintain indexes of the state of periodontal tissues at the same level after the conservative treatment. The deterioration of oral hygiene indices in the comparison group in 12 months after the treatment caused a deterioration of the indicators of the state of periodontal tissues (periodontal indices, depth of periodontal pockets, and the level of clinical attachment). The results of the research prove the high efficiency of the application of the developed universal folding periodontal splint. Its use contributes to a significant improvement of the oral hygiene and the state of periodontal tissues, compared to adhesive splints, promoting stable stabilization of the pathological process in the long term of observation.
