The work is dedicated to optimization of diagnosis and prediction of the course of the disease in patients with chronic obstructive pulmonary disease (COPD) with concomitant hypertension (HT) based on the determination of biomarkers - interleukin (IL)-18 and IL-10. Patients with COPD in combination with HT in comparison with patients with isolated COPD have more pronounced activity of systemic inflammation and compensatory response with increasing IL-18 and IL-10. Desaturation during physical activity is associated with an imbalance of inflammatory response with increasing proinflammatory IL-18 and decreasing anti-inflammatory IL-10. It has been determined that the level of IL-18/IL-10 ratio is a prognostic marker of adverse disease course in patients with COPD in combination with HT. The growth of IL-18/IL-10 ratio is associated with the worsening of patients with comorbid pathology with exercise intolerance, increasing breathlessness, decreasing saturation while walking, remodeling bronchopulmonary system as emphysematous changes, signs of overload right heart by pressure and decreasing quality of life. The Clinical COPD Questionnaire (CCQ) provided predictive information on the course of the combined pathology.
