Vasko A. Substantiation of expediency of neoadjuvant chemo-ray therapy in patients with low-differentiated cancer of the thyroid gland

The thesis is devoted to the study of the characteristics of low-differentiated thyroid cancer and to improve the treatment outcomes of these patients by optimizing the therapeutic tactics by conducting neoadjuvant chemo-radial therapy. The morphological signs and the panel of informational markers for sharp-differentiated thyroid cancer on the preoperative stage of diagnosis by means of conduction of a trypan-biopsy have been determined, which allowed to develop and carry out clinical approbation of the method of neoadjuvant chemo-radial therapy in this category of patients. The application of the developed treatment method to achieve the ablative effect and reduction of the tumor allowed surgical treatment to be carried out in the most radical volume in 65% of cases, which reduced the relapse rate by 34.80% and increased the survival rate of patients by 27.15%. The program of diagnostics, treatment and post-surgical supervision of patients with low-differentiated thyroid cancer, including trypan-biopsy of the tumor, neoadjuvant chemo-radial treatment, total removal of thyroid gland and metastatic lymph nodes, and subsequent follow-up of patients with thyroid-stimulating hormone levels, thyroid-stimulating hormone antibodies, neck ultrasound, computed tomography of the chest and / or magnetic resonance imaging, have been developed.
