Essay

The dissertation is devoted to the determination of the indications for coronary artery bypass grafting on a working heart under artificial blood circulation based on a multifactorial analysis of preoperative risk factors for the occurrence of intraoperative and postoperative complications. For the purpose of the study, a retrospective analysis of the history of the diseases was performed on 4068 patients who had isolated aortic coronary and / or mammary coronary artery bypass grafting (CABG), including 82 (2,0%) patients with urgent conversion to the on-pump during CABG and 110 (2, 7%) of patients who had a planed On-pump CABG on beating heart (without cardioplegia). General clinical data, features of concomitant and basic pathology, anatomical and pathological features of the coronary arteries structure and surgical intervention are analyzed. The most important predictors of intraoperative and postoperative complications were determined and a system of prediction of their development was proposed, which allows to identify patients with whom it is advisable to plan On-pump beating heart CABG.
