Masik O. Clinical aspects and treatment of mental disorders in adolescents with somatic diseases

The research under the thesis was carried out on the basis of the study of maladjustment, parenthood and family relations, and psychopathic symptoms aimed at examination of peculiarities of formation, clinical development and progression of mental disorders in adolescents with somatic diseases, increase in efficiency of treatment and gradual medico-psychosocial rehabilitation via optimization of therapeutic pharmacotherapy complex in association with psychotherapeutic component for which the following research methods were applied: social-demographic, clinical-psychopathological, pathopsychological and statistical methods. The total number of adolescents that participated in the research, on condition of their informed consent, made 329 people, patients of Children's Regional Hospital of the city of Khmelnitsky, 90 adolescents from Children's Regional Hospital and Psycho-neurological Hospital of the city of Vinnitsa. 27 patients from Group №1 under the research received standard treatment with anti-depressants; 30 patients from Group №2 underwent therapy with anti-depressant and normothymic; 33 patients from Group №3 under the research underwent therapy with anti-depressant and normothymic and psycho-terapeutic intervention. After implementation of the goals of the research and achievement of the objectives of the thesis, the patients underwent full three-component psycho-therapeutic treatment in addition to standard pharmacotherapy. Application of the therapeutic complex that we developed (client-centered psychotherapy combined with psycho-pharmacological correction) make it possible to recover compensatory and adaptation abilities of the child, and to prevent chronization of the disease, it can also be applied as a method of secondary prevention in case of mental disorders related to stress. In its turn, identification of risk factors for psychosomatic disorders related to stress, make it possible to improve measures aimed at primary prevention of this pathology. The thesis provides theoretical basis and new solution to the problem of psychosomatic disorders in adolescents. On the basis of multi-modal study, there were determined clinical-psychopathological peculiarities of syndromogenesis of mental disorders in adolescents with somatic diseases from various social groups, as well as prognosis criteria for maladjustment; there was developed a complex of preventive measures at different stages of specialized help.
