The dissertation is devoted to solving the urgent task of modern ophthalmology - to increase the effectiveness of pterygium treatment by identifying the factors that lead to its occurrence and complications after surgical treatment in the form of relapses. The research contained 203 patients (232 eyes) with pterygium underwent surgical treatment with various traditional methods and our modified method, and the frequency and features of its recurrence within 1 year were studied. Molecular genetic studies have been carried out in the removed pterygium tissues. For the first time was determined the dependence of pterygium recurrence after surgical treatment on the presence of the BRAF V600E mutation: the presence of the BRAF V600E mutation of the BRAF gene increased the risk of recurrence during the year after surgery 35.7 times. Was developed a modified surgical treatment technique of pterygium, which reduced the incidence of pterygium recurrence by 17.0% during the first year after surgery (χ 2 = 5.32; p = 0.021). In the presence of the V600E mutation of the BRAF gene, the use of the developed surgical method reduced the recurrence rate by 39.8% compared to traditional methods. Constructed a mathematical model of multivariate logistic regression of the probability of recurrence of surgical treatment of pterygium depending on the presence of the V600E mutation of the BRAF gene and VEB. The model had satisfactory properties.
