Essay

Object: patients with multiple fractures of long bones of different age groups. Aim: to improve the results of surgical treatment of patients with multiple fractures of long bones with implantation into the defect of β-tricalcium phosphate Biomin TGG-2. Methods: general clinical, radiological, computer, densitometric, electron microscopic, histological, mathematical modeling, biochemical, statistical methods and functional tests. For the first time in the experiment, the regeneration of the femur of rats under the conditions of implantation of β-tricalcium phosphate (Biomin TGG-2) into the diaphyseal and metaphyseal defects was studied. Biocompatible and high osteoconductive qualities of the material have been proven, as evidenced by the location of the cells and the formation of bone tissue on the surface of the granules. Bioresorption of granules occurred due to the action of macrophage diferon' cells, tissue fluid and the germination of osteogenic fibrous tissue. At the end of the study (day 56), the area of granules in the defect exceeded 60% of its territory, which reflects the low bioresorption activity of the material and allows it to be used to fill large bone defects. For the first time, based on the analysis of the maximum values of equivalent stresses on the developed mathematical models using the finite element method, the effectiveness of using β-tricalcium phosphate Biomin TGG-2 for replacing bone defects in order to restore its integrity was proved. On the basis of a prospective study in clinical groups of patients with multi-fragment fractures of the limb bones, it was proved that the use of β-tricalcium phosphate Biomin TGG-2 for plastics of bone defects in combination with osteosynthesis in surgical treatment allows to obtain 1.12 times more good results compared to osteosynthesis without replacement of defects. In an in-vivo experiment and with the help of mathematical modeling using the finite element method, the expediency of using β-tricalcium phosphate Biomin TGG-2 for the plastics of bone defects was substantiated. Clinical testing has confirmed the effectiveness of using the material under study for replacing bone defects in combination with osteosynthesis in the surgical treatment of patients with multiple fractures of limb bones. Osteoinductive and osteoconductive qualities of the material will allow to extend the indications for the minimally invasive use of β-tricalcium phosphate Biomin TGg-2 in elderly patients with concomitant diseases at the stages of control with predictive risks of disruption of regenerative osteogenesis of the clinic of multiple ulcers. to introduce in practical traumatology and orthopedics in the form of methodical developments and information sheets for use in treatment.
