he dissertation is devoted to solving the urgent scientific problem of traumatology and orthopedics - to improve the results of treatment of patients with gout, with gouty arthritis and tophus gout by developing the newest and improving existing methods of modern orthopedic treatment. The basis is the analysis of diagnosis and treatment results of 111 patients with established according to the criteria diagnosis Wallace et al. the acute gouty arthritis and chronic tophus gout, who were examined and treated in the departments of the State Institution "Institute of Traumatology and Orthopedics by the National Academy of Medical Sciences of Ukraine" (Kyiv) from 2009 to 2018). Patients' age ranged from 30 to 80 years (mean age 58.4 ± 5.3 years). Patients were 100% male. The distribution of patients by groups, age, length of disease, number of relapses was estimated by Pearson's criterion (χ2) and was the same (p> 0.05). Depending on the course of the disease, all patients were divided into two clinical groups, which were divided into two subgroups. The first group included patients with acute gouty arthritis (53 patients). The main subgroup of the first group included patients receiving treatment according to our developed method (27 patients), the control group - 26 patients with acute gouty arthritis receiving treatment by other conventional methods and techniques. The second group included patients with chronic tophi gout (58 patients). The main subgroups of the second group consisted of patients who underwent surgical treatment, according to our developed indications (27 patients), to the control - 31 patients with chronic tophi gout, who did not have surgery. On the basis of histomorphological researches, the stage of development of morphological pathological changes in paraarticular tissues and tissues of the joint of patients with gout was determined. The classification of tophi maturity was first developed according to the sonographic study of gout patients. Based on sonographic and pathomorphological studies of soft tissue in patients with gout, we have substantiated indications for surgical treatment.
