Essay

The study included 106 patients with stage IIA-IV colorectal cancer with severe bleeding, on the background of a common tumor process of the rectum who were treated at the State institution "V.T. Zaycev Institute of general and urgent surgery of National academy medical sciences of Ukraine" from 2015 to 2020. All patients were divided into two groups - the main (61 patients) and the comparison group (45 patients), depending on whether only surgery was performed, or it was preceded by a stage of endovascular hemostasis. Based on the developed algorithm, the method of endovascular hemostasis was chosen for patients of the main group: intra-arterial chemoembolization with oxaliplatin or intra-arterial occlusion using Gianturco coils or gelatin sponge (Gelfoam™). Technical success in the use of endovascular hemostasis was 95.09%. Postoperative mortality was 20% in the comparison group, the postoperative mortality rate was 1.68% in the main group. When analyzing the long-term results of treatment of patients in the control group found: one-year survival without progression in the main group was 68.33% in the comparison group - 44.44% (p<0.05). When analyzing the immediate results of treatment of patients in the control group, it was found that the one-year overall survival in the main group was 81.67%, in the comparison group - 58.33% (p<005).
