The purpose of the thesis was to improve the quality of endodontic treatment of periodontitis by developing a method of using ozone for chemo-mechanical treatment of root canals, depending on the clinical forms of periodontitis. The analysis of orthopantomograms showed that the prevalence of complicated forms of caries is 85.8% with an intensity of 4.15 teeth per one examined. The need for endodontic dental treatment is 10.5% of the total number of teeth, or on average 2.9 teeth per one examined. More than 50% of endodontically treated teeth must be retreated. It was found that the use of an improved irrigation technique allows achieving better results of cleaning the dentin of the root canal, both in general and its apical part. Microbiological examination of the material obtained from the root canals of the teeth of patients with chronic apical periodontitis isolated 147 strains, of which aerobic - 52, facultative anaerobic - 31 and obligate anaerobic microorganisms - 64, including 92 strains of gram-positive cocci. In chronic forms of apical periodontitis, anaerobic microorganisms were sown more often (45.3%), and with its exacerbation, aerobic flora prevailed (39.5%). Based on the data of microbiological studies, it was revealed that after thorough chemo-mechanical treatment of the root canal using ozone in chronic apical periodontitis, the number of isolated strains decreased by 72% (3.5 times). On the basis of experimental laboratory studies and clinical observations, the most effective algorithm for drug treatment of the dental root canal system has been substantiated and implemented, it allows to improve the quality of their cleaning, increase the success of endodontic treatment and reduce to a minimum the number of complications of dental root canal treatment (6% sodium hypochlorite solution, ozonized water and an ultrasonic Endoactivator (Dentsply) for 1 minute) during the entire chemomechanical treatment step. Analysis of the immediate and long-term results of treatment of complicated caries made it possible to establish the clinical efficacy of the proposed method of chemo-mechanical treatment. Successful results were achieved in the treatment of chronic apical periodontitis after 24 months in 97.7% of clinical cases.
