Essay

Subject of research is the process of union of fragments at stable-functional osteosynthesis. The purpose is to work out a new direction in the system of methods of curative surgery for fractures - low-invasive osteosynthesis, which main goal is the highest possible maintenance of blood circulation in the area of fracture. Methods of research - іn the process of work clinical, roentgenologic, histomorphological, mathematical, and biomechanical methods of research were used. The results of treatment of patients were evaluated by Matisse system. Multivariate comparative correlation-regression analysis of fracture treatment results was carried out. Received data were processed with method of comparative statistical analysis with use of Student's factor, with method ? (Fisher's angular transformation) and with use of personal computer. New knowledge: the experience of use of methods of traditional and low-invasive osteosynthesis on the basis of evaluation of results of therapy has been generalized. For the first time, a new direction of surgical treatment for long bones fractures was grounded and worked out, the basis of which is low- and minimally-invasive osteosynthesis. We have developed and put into practice a minimal contact plate that provides maintenance of periosteal vascularization of fragments, intramedullary fixation device that ensures optimal rotation stability, and hollow screws for drainage of intramedullary canal and intraosseous introduction of antibiotics. We have determined indications and terms, developed and introduced an atraumatic technique of use of methods of low-invasion extracortical, intramedullary and external osteosynthesis of long bones in case of multifragmental and segmental fractures. We have worked through and specified indications for use of methods of traditional and low-invasive osteosynthesis. We have put into practice a complex anatomic-functional system of evaluation of severity and forecasting of course of multitraumas. We have worked out, confirmed and put into practice the tactics of initial and distant osteosynthesis of fractures of long bones and hip bones, peculiarities, terms and stages of its realization. We improved and worked through the systems of treatment of patients with open and closed fractures. We proposed the system of rehabilitation of patients after traditional and low-invasion osteosynthesis of long bones and hip bones fractures, terms and stages of rehabilitation measures for renewal of full life quality of patients. Approbation of developed system of methods of low-invasion stable-functional osteosynthesis in the Traumatic Surgery Clinics of Kyiv Clinical Emergency Hospital on 4712 patients has shown high efficiency and economical profitability thereof, which allows to recommend that system of methods for broad introduction to the healthcare practice. Practical importance: We developed new scientifically grounded fixation devices that comply with requirements of biological concept of stable-functional osteosynthesis, worked out a differentiated approach to the choice of tactics of treatment and rehabilitation of patients with fractures of bones. We worked out a new plate, which use preserves vascularization of the bone due to limited contact of the implant with the bone. We created a nail for intramedullary osteosynthesis that insures optimal rotation stability of fixation of bone fragments, enables carrying out functional treatment from the first days after the operation. We improved and put into practice the external fixation device that allows to improve the results of treatment of patients with open and closed fractures, reduce the number of suppurative complications as well as the term of patients' stay in the hospital. We have developed the schemes for diagnostics and specialized traumatologic aid in case of isolated traumas and multitraumas, which has allowed to choose an optimal time and method of surgical treatment of injured patients, contributed to reduction of terms of in-patient treatment and improvement of functional results.
