Yermolayeva M. Lung Disorders in Patients with Systemic Connective-Tissue Diseases: Diagnosis, Pathogenesis, Treatment (a Clinicoexperimental Study)

Subject of research - patients with systemic diseases of connective tissue (systemic lupus erythematosus (SLE), systemic sclerodermia (SSD), rheumatoid arthritis (RA)) with the affection of lungs and animals with a model of autoimmune disease; aim - the determination of pathogenesis and the improvement of quality of early diagnostics and the effectiveness of treatment of pulmonary affection in patients with systemic diseases of connective tissue; methods - clinical, laboratory, instrumental, radiological, biochemical, physics-chemical, immunological, immunoenzymatic, morphological, statistic; newness - respiratory humor aquosus excretion, physics-chemical properties of expirates, pulmonary synthesis and respiratory excretion of unalbuminous nitrous compounds, lipids, lipoproteins, products of lipid peroxidation and antioxidative protection in patients with systemic diseases of connective tissue have been determined for the first time, its importance in early diagnostics of pneumopathies and a bond withthe function of bronchopulmonary system have been established, the disorder of pulmonary surfactant system in patients with SLE, SSD and RA has been proved, the using of stimulators of pulmonary surfactant's synthesis and syngletnicooxygen therapy in complex treatment has been grounded, the development of pulmonary intoxication in these patients has been established, the interconnection of changes of some morphological structures of respiratory system in patients with SLE with the peculiarities of clinic- laboratory course of disease has been demonstrated, the morphological changes of alveoluses, bronchuses and pulmonary vessels, pathogenetic influence to them by immune and nitric oxide disorders, pathomorphosis under the action of glucocorticosteroids and cytostatic drugs have been determined on the experimental model of autoimmune disease in animals, the role of disbalance of pro- and antioxidant systems, cellular and humoral immunity with the development of delay-type hypersensibility, and as well pulmonary synthesis of proinflammantory and regulative cytokines in pathogenesis of pulmonary affection in patients with systemic diseases of connective tissue has been proved; results - the medical technology of diagnostics and treatment of the affection of lungs in patients with systemic disease of connective tissue has been devised, it has allowed to increase a quality of early diagnostics of pulmonary pathology, to pick out the criteria for estimation of pneumopathy's severity in patients with SLE, SSD, RA and for prognosis of the effectiveness of treatment, to offer the stimulators of pulmonary surfactant's synthesis and syngletnicooxygen therapy as therapeutic measure; it has been introduce in practice of therapeutic-preventive departments of Donetsk, Lugansk, Zaporozh'e and Kharkov Medical Universities; sphere of using - medical-diagnostic process of rheumatological, pulmonological, therapeutic departments, a teaching process in therapeutic sub-faculties of medical universities.
