Essay

On the basis of integrated examination of 285 children and adolescents with a severe form of diabetes mellitus (DM) there were revealed certain mechanisms in formation of somatopsychic and psychosomatic correlations in such patients, as well as the incidence and structure of clinico-psychopathological disorders. There have been established some psychological peculiarities in the patients as regards intellectually-cognitive, motivational and emotional spheres. It has been shown that at early stages of DM in children the progressive diabetic encephalopathy (DE) was formed. Under conditions of modeled acute "expectation stress" there have been established in the patients certain exhaustion of the organism functional resources and the failure of antistress adaptability mechanisms in the brain. There have been worked out an algorithm of diagnosis and prognosis of psychopathology in children with DM and also a concept model of DE pathogenetic therapy. There has been formulated a hypothesis on psychoprophylaxis of maladjustment, and a specific clinical trend in psychiatric rehabilitation in children's diabetology has been grounded.
