Essay

The thesis is devoted to the determination of forming and development mechanisms of cardiovascular complications in patients with diabetes mellitus type 2 and various manifestations of metabolic syndrome. On the basis of complex investigation of the Kharkiv region population and groups with high risk for diabetes mellitus, arterial hypertension and metabolic syndrome, the prevalence of 4 and 5-component metabolic syndrome and its separate constituents were studied. The influence of each component of metabolic syndrome on the development of cardiovascular complications in the patients with diabetes mellitus type 2 was stated and the most unfavorable cardio-damaging combination of them was determined. The presence of direct connection between the degree of dysglycemia and atherosclerotic peculiarities of structural and functional changes in left ventricle myocardium in patients with diabetes mellitus was grounded. The essential contribution of immune-inflammatory cytokine link according to tumour necrosis factor-? and interleukin-6 in the development of concentric and excentric myocardium remodelling, in the decrease of ejection fraction and severity of heart failure was proved. Results of investigation gave possibility to ground the modern strategy and tactics of complex therapy for the cardiovascular complications in patients with type 2 diabetes mellitus and various manifestations of metabolic syndrome depending on amount of constituents and duration of the disease.
