Essay

The dissertation is devoted to the important scientific-practical issue in pediatrics – improving of diagnostic and curative health care approach for newborn with perinatal infection disorders and disbalance in adaptation. The improvement will include implementation of diagnostic algorithm, clinical and laboratory criteria of prognosis and new curative approaches. Thesis are based on the observation over 231 newborns with perinatal infection pathology clinical signs, 113 patients with adaptation disbalance in early neonate period, and 25 healthy newborns. It is shown the integral clinical-paraclinic and morphological characteristics of perinatal infection pathology in newborns, and the independent predictors of unfavorable income were selected. It is also shown the interrelationships between the complex evaluation of the homeostasis with introducing of pathogenethic heterogenic immune disorders, signs of membrane destructive processes intensification, metabolic intoxication in newborns with infectionpathology taking into account perinatal risk factors, morphological and morphometric status of the fetal placental complex, and structural organization of the peripheral blood lymphocytes. Differentiative models of multifactorial prediction of unfavorable outcomes in newborns with adaptation disorders and in newborns with clinical signs of infection pathology with lethal outcome were developed. These models allowed finding the patient groups to be influenced in a stronger way depending on their risk factors.
