Kvachenyuk A. The particularities of diagnostics and treatment of adrenal malignant tumors

Object: malignant adrenal tumors (MAT) of children and adults, trend of disease and tumor’s spreading. Purpose of research: to analyze the clinical trend and hormonal characteristics of MAT, to analyze the methods of diagnostics and treatment of patients with MAT, and with this basis to create applied algorithm for management of these category of patients, to follow it to lead the increasing of curative effectiveness and prolongation of survival. Methods of research and the equipment: clinical, laboratory, instrumental, morphological, mathematics; microscopes Nicon, Olympus, ultrasound Toshiba 240S, CT-9000HP.Theoretical and practical results: In the theses it was done the theoretical ground and new scientific solution of actual problem of modern endocrinology – increasing of curative effectiveness for MAT (improving of patient’s survival, decreasing of recurrences, regional and remote metastases) with the optimization of diagnostic and curative procedures and create the applied algorithm for management of patients. With the help of creation of new methods of surgery with lymphatic dissection and schemes of chloditane-therapy, in comparison with known in literature, to obtain the mot effective results of combine treatment for MAT Novelty: for the first time it was done the complex analysis of methods of imagine diagnostic (ultrasound, CT, MRI, macroscopic view) of adrenal tumors and first it was systemized and clearly determined the criterias of malignancy. First time in Ukraine it was inculcated the methods of morphological pre- and intraoperative verification of tumor’s nature, which could be used for the majority of patients with high level of informative ability of obtained materials. First time it was investigated the results of use of different kinds of surgery and first time it was proposed the surgical intervention on the regional collectors of lymphatic drainage, which supply the most effective results for treatment of MAT. Degree of introduction: Results can be used in diagnosting and treating the patients with MAT for children and adolescence. Area of using: endocrinology, surgery, oncology.
