Dissertation covers optimization of approaches to medical management and treatment of HIV-infected children with different variants of natural course of the disease. Course of HIV disease in perinatally infected children on HAART was studied. Clinical manifestations of HIV infected were observed in cohort of 207 HIV-infected children. Diagnostic effectiveness of clinical manifestations and laboratory data was evaluated. Conditional peculiarities of immune system of HIV-infected children with different variants of disease progression were discovered. Factors that influence the speed of HIV progression in perinatally infected children were studied by mono- and multifactor analysis. Psycho-social problems that occur during medical management of HIV-infected children were found and their role as a factors of disease progression was defined. It was proved that social factors influence the risk of treatment failure. Complex differential approach to medical management of HIV-infected children was created. This approach is based on evaluation of speed of disease progression, necesserity of social support.
