Ponomarev I. Combined treatment in organ-sparring surgery in patients with early stage breast cancer.

This work is devoted to development of the program of combined treatment of patients with early stage breast cancer, performing organ-sparing surgery, and including new tactics of preoperative irradiation – local tumor irradiation for more effective treatment results. The application of preoperative irradiation in dose 25 Gy to the tumor and 15 Gy to the whole breast with consequent organ-sparing surgery decrease the local relapse-rate in patients younger than 55 years old in 1,7 times, in menopausal women – 3,9. The increase of local irradia-tion dose from 25 to 34 Gy to the tumor and from 15 to 25 Gy to the whole breast, before organ-sparing surgery, decreased the local relapse-rate in patients younger than 55 years. In women older than 55 years there were de-tected no relapse. This fact conducted to the tumor regression at tissue level in 85,7% of patients; decrease of tumor mitotic activity in 6 times in comparison to tu-mors, which were not irradiated, and increase of quantity of pathologic mitoses in 1,8 times. This developed program of combined treatment allowed to increase the efficacy of treatment results in these patients, and is to be applied for clinical practice in oncological clinics.
