Essay

The present thesis is centered around the theoretic substantial and new solution of one of the principle neonatology problems - creation of the latest diagnostics and prognosis concept of gastrointestinal disorders in premature newborns with perinatal pathology on the basis of determining clinicoparaclinical, immunocytochemical, immunal, microbiologic and morphologic criteria for pathogenetic mechanisms of necrotizing enterocolitis (NEC) development, elaboration and introduction of advanced methods of early diagnostics and prognosis at different stages of the disease. By complex researching into premature newborns bowels and their pathomorphologic examination there have been developed functional state features of the gastrointestinal system during neonatal period pathology in comparison with findings of abdominal radiography and bowel ultrasonography, mesenterial blood flow doppler ultrasonography, non-specific immunity factors, neutrophil functional activity and subpopulation, intestine microbiocenosis, lymphocyte apoptosis and necrosis levels of peripheric blood, morphologic and pathomorphologic data. Due to the research carried out there have been created correlation pathogenetic and pathomorphologic patterns of NEC development, the system of early diagnostics and prognosis of gastrointestinal functional state disorders in premature newborns with early neonatal pathology, including the mathematic model and computer software for NEC diagnostics and prognosis which offer the opportunity to form risk groups of the disease development and take timely preventive and therapeutic actions.
