Essay

The dissertation is devoted to the study of mechanisms in the development of myocardial electrical inhomogeneity in hypertensive patients depending on age, sex and comorbidity in the form of diabetes mellitus and cerebral vascular pathology. The frequency of late and early ventricular and atrial potentials detection was determined and sex and age differences were revealed in the character of heart electrical heterogeneity both on healthy subjects and hypertensive patients suffering from diabetes mellitus and having undergone acute ischemic stroke. Essential hypertension and diabetes mellitus alone appear to be main determinants for electrical heterogeneity development in atriums, while comorbidity of arterial hypertension with diabetes mellitus and/or cerebral vascular pathology was responsible for forming ventricular electrical inhomogeneity. Discrepancy between the heart hypertrophy assessed by ultrasound and electrocardiogram is associated with an increase of heart electrical inhomogeneity; and related indexes elaborated were demonstrated to be informative as indirect markers of it. Well-controlled antihypertensive and hypoglycemic therapy were shown to be essential prerequisites for heart electrical homogeneity improvement.
