Object of the study: Metabolic syndrome (MS). Aim of the study: to work out an algorithm of MS differential treatment in patients with type 2 diabetes mellitus (DM2) under iodine deficiency at the Ukrainian Carpathian region on the basis of studying peculiarities of pathogenesis, clinical course of MS, role of iodine insufficiency and hypothyroidism in the development of MS. Methods and equipment: clinical, biochemical, immunological, immunofermental, instrumental, statistical; apparatus "Elegra" Siemens Medical Systems, test-system D-10 TM, analyzer "Stat Fax-303", PC Pentium-IV. Theoretical and practical results: a new pathogenetic concept of the developing hypothyroidism associated metabolic syndrome in patients with DM2 was formulated on the basis of studying clinical course and pathogenesis including indices of humoral, immunologic mechanisms, hormones of fatty tissue, proinflammatory cytokines, functional state of the thyroid gland and the liver. The efficiency of diagnosis and treatment of DM2 with MS under iodine insufficiency was increased. Novelty: for the first time, the clinico-biochemical examination of the MS course peculiarities was realized in the Ukraine, an algorithm for diagnosis and treatment in patients with DM2 under iodine deficiency of the Ukrainian Carpathian region was worked out and introduced. It was established that decreased level of adiponectin and increased level of resistin in blood serum under iodine deficiency had to be consider as markers of MS. Degree of implementation: the findings were patented, methodic recommendations for diagnosis and therapy of hypothyroidism associated metabolic syndrome and immunocorrection in patients with DM2 and MS under iodine deficiency were published, innovations were worked out. Materials of dissertation were included into the chapter of manual for the students of medical institutions. Area of using: endocrinology, family medicine.
