The work covers the problem of improvement of diagnosis efficacy at chronic kidney disease (CKD) progress to chronic renal failure in children on the basis of clinical and pathogenetic patterns and structural functional disorders of kidneys and heart revelation, creation of pathological process development concept and working out the diagnostic tactics algorithm at various stages of clinical monitoring. The risk factors of CKD progress and forming cardiovascular complications, their degree of influence at different CKD stages were determined. The additional markers for diagnosis of nondiabetic and diabetic CKD were defined on different stages of the process. The correlation between renal structural functional changes and cardiovascular status parameters at different stages of CKD progress with renocardiac continuum forming was analyzed. The activities of vasoconstrictive, apoptogenic, destructive proteinases, factors influencing extracellular matrix expression and their relations with central and renal hemodynamic parameters were studied. The investigation program for CKD children was developed; its expediency for an early CKD diagnosis was scientifically substantiated.
