Essay

Object: lacrimal gland epithelial tumor genesis. Purpose: the efficacy rising of lacrimal gland epithelial tumors complex treatment on the basis of clinical-morphological, computer tomography and immunological features studying, which allow to carry out differential diagnostics of malignant tumoral degree, to prognosticate its relapse and also survival rate of patients. Methods: palpation, computer tomography and nuclear-magnetic resonance, histomorphological, ultrastructural, cytophotometric, immunohistochemical, immunocytochemical, immunofermental methods, methods of the statistical analysis. Equipment: tomographs "Somatom CR", МRТ-50А, "Magnitaime", microcytophotometr МCPВ-1, electronic microscope PEМ-100. The ultrastructural features of pleomorphic adenomas of lacrimal gland have been specified. The prognostic hystomorphological criteria of relaps of ephithelial lacrimal gland's tumor were added. The knowledges about antitumor immunity at lacrimal gland tumors were expanded; it was shown that the gravity of tumoral process was connected with immunodepression. It has been specified that antitumor immune response also was connected with high level of expression of CD7+, CD38+, CD45+, CD150+. The scientific data that level excess oncomarker СА-19-9 in blood of patients with lacrimal gland neoplasma testified to presence of tumoral process was added. The knowledges about immune-dependent character of lacrimal gland tumors has been confirmed by infringement of immune reactions which corrected by induction of interferon of a-, b-, y- types. Data that tumors of lacrimal gland botyh benign and malignant character have a dense consistence, a smooth surface, more often the motionless was specified. Different diagnostic value of computer-tomografic semiology of lacrimal gland tumors was specified. Criteria of differential diagnostics of lacrimal gland tumours at computer-tomografic research were added by definition of tumor dencimetria. Criteria of differential diagnostics of character of lacrimal gland tumors based on analysis of blood with CD7 +, CD38 +, CD45 +, CD150 + were expanded. For the first time the method of forecasting of relapse of tumors of lacrimal gland based on analysis of blood with CD16 +, CD25 +, CD95 + was developed. It was developed the prognostic test of a joint estimation of level of the maintenance in blood of patients the oncomarkers СА-19-9 and REA which allows to predict tumor relapse. There were established the prognostic factors of survival rate for patients with lacrimal gland tumors. Devised method of the combined treatment with inclusion of peroral reception of amixin was developed. Results of work were introduced in practice at SI "The Filatov Institute of Eye Diseases and Tissue Therapy", the diagnostic centre of Open Company "Isma-Invest".
