The dissertation work contains a new approach to solving scientific problems in the military medicine, which provides improving effective prevention, diagnosis and treatment of lower limb bones' stress damages among the servicemen of the Armed Forces of Ukraine. The research is based on a complex analysis of the statistical, anamnestic, clinical, biomechanical data, results of radiological methods of diagnosis and treatment of 230 servicemen with stress damages of the bones. The research determined the level and structure of stress damages of the bones among the all categories of illness and diseases of the musculoskeletal system among the servicemen. Accurate reasons and direct correlated correspondence of the beginning of bones' stress damages were discovered as a result of different types of physical activity, terms of the military service, static and axial deformation of the bones and joints of the lower limb. Clinical and radiographic classification of stress damages of the bones. has been developed. This classification includes: mode (typical, sclerotic, cystic-lacunary), staging (hyperostosis, pathological rebuilding of the bone, stress fracture) and clinical course (acute, chronic relapsing) of the rebuilding process in the bones. The algorithm for the initial stage diagnostic of bones' stress damages by the methods of radial diagnostics has been proposed. The algorithm determines the optimal sequence of application depending on the location, shape and stage of rebuilding process in the bones. The biomechanical features of walking in the period of the lower extremity bones' stress damages using and without individual correcting insoles were scrutinized. Recommendations of using individual correcting insoles have been proposed for the treatment and preventive measures. Increasing of subfascial pressure in the musculoskeletal cases of the lower limb damaged segment has been diagnosed. This indicates a chronic compartment syndrome, which bring to disruption of the intraosseous and regional hemodinamics and component of the pathogenesis of stress damages of the bones. The diagnostic and treatment algorithm for the stress damages of the bones has been developed. The algorithm allows realization of the complex differential approach in a choosing of the methods of treatment and rehabilitation, depending on the location, shape, stage and in the course of rebuilding process in the bones. Systematic recommendations for the military-clinical examination were justified. The system of corresponding preventive measures for the servicemen of the Armed Forces of Ukraine has been developed and implemented. Key words: stress damages of bones, hyperostosis, pathological rebuilding of bone, stress fracture, classification, diagnosis, treatment, prevention, serviceman.
