The thesis is devoted to the investigation of the peculiarities of the clinical course, the changes of humoral factors, the functional state of the cardiovascular system, the establishment of prognostic criteria, the substantiation of the optical schemes in the treatment of Q-wave myocardial infarction in patients with metabolic syndrome. It is established that in patients with Q-wave myocardial infarction with metabolic syndrome in acute period there occurs the increase of lethality and complications, the desaptative character of postinfarction remodeling processes, the decrease of time and spectral data of the variability of heart rate, disorders of blood pressure circadian rhythm, metabolic and humoral changes. There is proved correlations between metabolic syndrome components and the course of the disease, humoral factors, characteristics of the functional state of the cardiovascular system. There are substantiated schemes of rehabilitation treatment by means of combined hypolipidimic, metabolic antiaggregatory and anti-diabetic therapy. There are determined the predictors of fatal and non- fatal coronary cases for 5 years of observation.
