Essay

A total of 104 patients with chronic thromboembolic pulmonary hypertension ( CTEPH) was examined. The mechanism of disease, its clinical course and anatomical types of CTEPH were studied. It was proposed to assess operability of lesions by reviewing clinical and haemodynamical data and radiologic investigations. Different scenarios of pulmonary endarterectomy and strategy of postoperative management have been proposed. The postoperative complications were assessed. The mortality rate has been reduced steadily, and was 5,5% since 2005 and no one died among the last 21 consecutive patients operated on since 2008. Pulmonary thromboendarterectomy provided remarkable early and late results in 90% of patients with CTEPH.
