The thesis is devoted to research and analysis of adipokines role as integral criteria of insulin resistance (IR), immune inflammation, prothrombotic state, type 2 diabetes, cardiac structural and functional dysfunction in association of arterial hypertension (AH) and obesity. The study involved 237 patients with AH. Pathogenic relationship between adipokines, lipids, carbohydrates have been detailed, it was found associations between them and echocardiography parameters, proinflammatory and prothrombotic state. Clinical and laboratory signs of adipokines dysfunction have been allocated that are early predictors of metabolic disorders with the formation of IR and type 2 diabetes. The results allow outlining the peculiarities of the course and progression of metabolic disorders on the basis of additional identifying of adipokines dysfunction as cardiometabolic risk factor. The effect of antihypertensive therapy on adipokines profile was detected that can be considered an additional criterion for selecting of treatment regimen. Relevant to results of the study a mathematical model of hypertension course prediction according to the most prognostically significant criteria changes such as anthropometric parameters, parameters of carbohydrate and lipid profile and adipokines in patients with obesity associated hypertension have been developed that allow allocate population and individuals with high cardiometabolic risk.
