The thesis is dedicated to the actual problem of pediatry - increasing of efficiency of chronic gastroduodenitis exacerbating course treatment in children, new approaches to exacerbation prophylaxis by study of pathogenetic mechanisms of gaster's and duodenum's mucosa chronic inflammation. Peculiarities of clinical course, acid production state and gaster's microbiocenose have been investigated in children with different nosological forms of gastroduodenitis in dependence on pathology duration and patients' age. Peculiarities of hystological and immunohystochemical changes of gaster's and duodenum's mucosa have been estimated in accordance with chronical gastroduodenitis' duration, childrens' age, gastric and duodenal inflammation form and activity degree. Gastric mucosa microcirculatory disorders and collagen formation in association with local immune regulation disorders have been proved as priority mechanisms of formation of gastric and duodenal mucosa inflammatory, erosive and atrophic changes. Melatonin regulative role in gastric cytoprotection, motor and acid production disorders has been investigated. It has been elaborated the protocole of antiexacerbative treatment of children with chronic gastroduodenitis, which should be prescribed on chronic gastroduodenitis in-patient treatment stage and will lead in gastric and duodenal inflammation exacerbation decrease.
