Skorohodova N. Differential diagnosis of infiltrative pulmonary tuberculosis and community-acquired pneumonia with delayed resolution

The object of study - the newly diagnosed previously untreated infiltrative pulmonary tuberculosis with negative sputum smear (IF NDTB), community acquired pneumonia with a delayed resolution (CAP DR); goal - improving the efficiency of the differential diagnosis of IF NDTB and CAP DR through the development of diagnostic algorithms; methods - clinical, radiological , microbiological, functional, endoscopic, cytological, immunological, immunoassay, statistics, novelty - established that for patients with CAP DR is characterized by a significant increase in cytokines, IL - 6, IL - 4, whereas for patients IF NDTB - a significant increase (especially in the bronchoalveolar lavage fluid) - TNF and IL - 2, developed a method of differential diagnosis of infiltrative processes in the lung with a negative smear on the basis of determining the concentration of TNF, INF, IL - 2, IL - 6 in culture supernatants of alveolar macrophages.
