The course and treatment results of 797 hip joints in 683 patients with hip joint osteoarthritis were investigated. 114 patients were treated with the conservative methods and 569 - were operated. Hip joint arthroscopy was conducted at 54 patients and 515 patient undergone hip joint arthroplasty. Three types of hip joint osteoarthritis according to the course of the disease were established - uncomplicated, complicated and mixed. Each type is characterized by the duration of the stage of the disease, amount of exacerbations per year, treatment results. The clinical manifestations of the disease relapse are pain and hip joint function disorders. The working classification of hip joint osteoarthritis has been designed, that allows to estimate the functional condition of hip joint, determines the choice of treatment, as is based on the ethiology and the stage of osteoarthritis, type of the course of degenerative-distrophic process, the state of compensatory processes, stages of exacerbation or remission, estimation of the hip joint functional condition, prevalence of the degenerative-distrophic process, and also X-ray examination results - the condition of bone, synoviai membraine, cartilage and correlations inside the joint. The relation of degenerative-distrophic process of the hip joint and cardiovascular diseases at patients with hip joint osteoarthritis based at the clinical and laboratory examinations results was established, that worsens the course of the disease and complicates treatment. Indications for hip joint arthroscopy at the early stages of the disease were worked out and this operation technique proved a great efficacy in the decrease of hip joint osteoarthritis worsening and hip joint function improvement. An algorithm of medical-social rehabilitation of patients with hip joint osteoarthritis was worked out that allows to improve surgical treatment results in particular to 81,6±1,1 points versus 74,3±1,3 points relatively (р<0,05), and also allows to specify the terms of administration to the МSЕC and to accept an expert decision. The results of treatment of 683 patients with hip joint osteoarthritis demonstrate that the differential approaches to the cure and rehabilitation of the patients taking into consideration the pathogenetic factors and the course of the degenerative-dystrophic process and timely administration of treatment allows to achieve positive results in the majority of patients. It was proved by the results of hip joint osteoarthritis treatment of patients from the basic group: excellent - in 15,2 %, good - in 47,3 %, satisfactory - in 32,0 %, unsatisfactory - in 5,5 %, that is much betteras compared with a control group: excellent - in 9,2 %, good - in 44,2 %, satisfactory - in 36,9 %, unsatisfactory - in 9,7 %.
