Based on the results of a comprehensive study of the relationship between clinical, metabolic, genetic, immune and morphofunctional units pathogenesis the concept of the role of genetic and environmental factors in the etiology of these diseases is formulated. Determined predictors of comorbid diseases: activation of free radical oxidation of depletion of antioxidant protection and increase of endogenous intoxication, allelic gene polymorphism phase II detoxification of xenobiotics, instability genotype, imbalance of cytokines, endothelial dysfunction. The new criteria for the preclinical diagnosis and prognosis of chronic obstructive pulmonary disease combined with hypertension were designed. The pathogenetic substantiation of application of domestic pharmaceutical means are given, proposed markers effectiveness of their use in chronic obstructive pulmonary disease and when it is combined with arterial hypertension and prevention of complications.
