The thesis is devoted to the improvement the level of diagnosis and individualized treatment of patients with hypertension combined with obesity on the basis of concept development pathogenetic role of adipose tissue hormones with the definition of adverse predictors of disease based on factors of immune inflammation glucometabolic disorders, markers of endothelial dysfunction, cardiovascular remodeling. It is proved adipokines value: resistin, visfatin, TNF-? and hormone ghrelin in the mechanisms of obesity and for the first time, the presence and sequence of hormonal dysfunction of adipose tissue as a failure of secretion and content of ghrelin in the blood of patients and hyperactivation of proinflammatory mechanisms of adipose tissue influence by visfatin, resistin and TNF-? increasing. Grounded differentiated approach to assign different options of combination antihypertensive therapy in patients with essential hypertension, depending on the state of the adipose tissue function and character of cardiovascular remodeling. The data on the relationship between medication reduced LVH and functional state of the heart, circulatory system and the blood adipokin's levels.
