Essay

Dissertation is devoted to improvement of diagnostics, treatment and prevention of chronic pancreatitis in children due to clinical peculiarities, functional condition of pancreas and liver, neuro-hormonal regulation between them. In the scientific work epidemiology, structure of pancreatic pathology in children and adolescents, peculiarities of clinical forms of pancreatitis in different age groups, risk factors for development of pancreatitis due to retrospective data of case histories and clinical examination were evaluated. Features of functional condition of the pancreas and liver in children with chronic pancreatitis and dysfunction of Oddi's spincter, pancreatic type, were examined. Role of pro- and anti-inflammatory cytokines in genesis of pancreatitis and dysfunction of Oddi's spincter, pancreatic type depending on severity and character of clinical course of the diseases was estimated. Violations of lipid and protein peroxidation in functional and organic pancreatic pathology were studied. Peculiarities of autonomic and neuro-humoral regulation between pancreas, liver and intestines in pancreatic pathology were found. On the basis of the risk factors mathematic model of prognosis of development of chronic pancreatitis was created. Complex of differential measures for therapy and prevention of chronic pancreatitis in children and adolescents with functional pancreatic pathology was created.
