Essay

During investigation of 484 patients with chronic pulmonary heart (CPH) and arterial hypertension (AH) we established, that the main mechanisms of this comorbidity formation are: neurohumoral and immune/inflammatory activation with endothelial dysfunction development and pathological apoptosis induction, increase of intrathoracic pressure and extrathoracic blood deponing. The differential prescription of enalapril and candesartan or their combination with S (-) amlodipine and nebivolol in patients with CPH and AH depends compensation or decompensation phase of CPH and chronic heart failure stage was justified.
