Essay

The thesis presents a theoretical basis and practical solution to the actual problem – improvement of the effectiveness of mandibular fractures surgical treatment. The study included the determination of biomechanical parameters of intact and damaged mandibles; development of advanced algorithms for computer simulation modeling of the mandible with a maximal approaching to the individual parameters of the patient’s masticatory system; investigation of the fundamental laws of stresses and strains distribution in the mandible and «fixator-bone» systems, which provides a grounded indications for osteosynthesis, and made it possible to define the principles of its application as well as the criteria of its effectiveness. The ways to optimize treatment strategy were determined on the base of comparative evaluation of osteosynthesis methods. The role and place of computer simulation methods in the complex of treatment of this type of injury was studied. The result of the present research was the development of the protocol for surgical treatment of the mandibular fractures, which took in consideration the present clinical, biological and biomechanical parameters. The study of immediate and long-term results of mandibular fractures treatment proved their medical, social and economic efficacy.
