It was evaluated the levels of tumor hypoxia, expressions of tumor microenvironment factors: Flt-1, СXCR4, c-myc, HIF-1a; VEGF: microvessels density, CD8-Т and CD45RO-Т lymphocytes in tumor tissue, influence of tumor microenvironment on the appearance of disseminated tumor cells, Flt-1 and СXCR4 expressions, CD8- Т and CD45RO- Т lymphocytes in bone marrow as well as determination of the lysophosphatidylcholin (LPC) concentrations and level of VEGF in blood sera by means of immunohistochemical, immunocytochemical and enzyme-linked immunosorbent assay methods and 31P NMR spectroscopy. It was confirmed that hypoxia of primary tumor negatively influences the efficacy of different methods of treatment: surgical intervention alone as well neoadjuvant or adjuvant chemotherapy and obtained the evidences for the expediency of the evaluation of hypoxia level in gastric cancer before the choice of therapy methods. Changes of LPC concentrations in sera found to be the most sensitive indicator of therapy efficacy applied and may be in use under individualized treatment in prediction of disease progression. It has been provided the analysis of investigated factors of tumor microenvironment associated with intratumoral hypoxia and some others indexes of immune system, shown their clinical relevance and working out the test-system for prognosis of disease outcome for the patients with gastric cancer.
