Essay

Object: evolution of scoliotic spinal curvature on the conditions of use of different types of surgical correction. Methods: clinical, radiological, biological experimental simulation, computed tomography, mathematical modeling, biomechanics, spirography, psychological, informational, analytical, statistical. For the first time found that the optimization process in osteoreparative tool fixing metal implant must exercise total discectomy combined with complete removal of end plates of the vertebral bodies and rebuilding bone graft in interbody gap reduces the stability of the fixation implant spinal motor segments. For the first time scientifically grounded application of low profile ventral implant for the surgical treatment of scoliotic spinal deformity. Established that fixing segments of thoracic and lumbar spine ventral implant compared with intact spine helps evenly distribution of the stress-strain state, reducing stresses in fixed vertebral bodies of the thoracic spine of 4,8-5,0 3,0-3,2 MPa to and in the thoracolumbar spine of 4,9-7,2 2,8-3,4 MPa to. For the first time using a physical model of "spine - implant" determined that the use of double-rod implants spine during the ventral correcting spinal fusion for correction of scoliotic spinal deformity is preferred for durability fixation versus single-rod - 57% in torsion, 61% in bending and 25% on compression. For the first time found that the application of an adjustment ventral and posterior spinal fusion saves normal levels sagittal contour of the spine and sacroiliac pelvic balance in the remote postoperative period. For the first time determined that in patients after ventral and posterior spinal fusion through an adjustment of compensatory-adaptive processes and dynamic optimization of patient body position in space marked improvement symmetry of the body for a long time after surgery. Supplemented scientific data on clinical and radiological signs interbody fusion after adjustment ventral spinal fusion that occur within 3-6 months. after surgery, as well as compensatory and adaptive changes in the spine for 9 months after surgical correction of scoliotic spinal curvature. Supplemented scientific data on mobility after spinal ventral and posterior spinal fusion adjustment in particular found that fixation implant leads to restriction of mobility of the spine, but to preserve the mobility of the spine using ventral instrumentation is preferred by 60% than the back compared with healthy subjects. It is found that posterior corrective spinal fusion versus ventral provides greater 17% correction of the main distortions, but also their use leads to an identical result of cosmetic treatments and body symmetry dynamic changes within 2 years after surgery. Found that regardless of the surgical approach of respiratory function in patients with scoliotic deformity of the spine after surgery restored to preoperative parameters after 2 years. The developed method of correcting the ventral spinal fusion, a way to determine the structural component deformation spine interbody fusion method in laboratory animals, mathematical model of thoracic and lumbar spine (patent of Ukraine №91696, №85647, №73817, №85805). The developed diagnostic and prognostic system that allows to justify the choice of treatment, to optimize the timing and tactics of treatment of these patients.
