Essay

The thesis is dedicated to the increase of efficiency of diagnosis, treatment and prevention of cardiovascular calcification in patients with chronic kidney disease by studying main risk factors, clinical and pathogenetic characteristics of cardiovascular remodeling course and modulation of the L-arginine–NO system as the therapeutic strategy of pathogenetic direction. It has been proved that endothelial damage in constellations with chronic inflammation is an important mechanisms of cardiovascular calcification in chronic kidney disease. High self-descriptiveness of circulating endothelial cells accumulation and decrease of flow mediated dilatation of brachial artery for early diagnosis of cardiac valve and carotid plaque calcification has been established. Efficiency and expediency of combined application of arginine glutamate and meldonium for preventive progression of valvular and vascular damage, optimization of clinical and hemodynamic status, improvement of the survival rate of the patients with chronic renal dysfunction has been rationalized.
