Object of the study: the process of renal dysfunction development associated with hypothyroidism. Aim of study: to improve the efficacy of treating patients with primary hypothyroidism by means of early diagnostics of renal dysfunction and to identify pathogenic mechanisms of their development on the basis of the study of inflammatory mediators, cytokine profile, functional state of the endothelium, lipid metabolism and fatty tissue hormones. Theoretical and practical results: The thesis reveals the frequency of renal malfunction in patients with hypothyroidism. 57.7% of patients with primary hypothyroidism have shown renal malfunction characterized by albuminuria, ?2-microglobulinuria, increased levels of creatinine in blood and decreased glomerular filtration rate in all groups of patients, more manifested in those with obesity. Prognostic factors of the development of renal disorders in patients with hypothyroidism have been identified. Morphologic peculiarities of the kidney in simulated hypothyroidism and obesity in terms of hypothyroidism have been studied. Efficacy of complex use of levothyroxine, inhibitor of angiotensin converting enzyme and atorvastatin in patients with hypothyroidism accompanied by renal malfunction has been investigated. Algorithms of diagnosis and treatment of renal dysfunction in patients with hypothyroidism have been developed and rationalized.
