Object of study: relapse of pulmonary tuberculosis and newly diagnosed pulmonary tuberculosis; aim - increasing efficiency prognosis, diagnosis and treatment of relapse of pulmonary tuberculosis by clinical, radiological, immunological peculiarities, influence of assessment modeling gene polymorphism of cytokines and C-reactive protein with the specific process; methods of investigation: general clinical, laboratory, radiological, microbiological, instrumental, immunological, genetic, mathematical, statistical; novelty: the first received information that relapse of pulmonary tuberculosis (RTB) is characterized by severe clinical and radiological course of disease; the expediency of prolongation phase of intensive chemotherapy for up to 4 months in patients with RTB with positive smear and up to 3 months - without positive smear; supplemented by scientific information for patients with RTB typical immunosuppressive course of the specific process due to changes for T-cell responses and decrease of the phagocytic activity of blood; the first received information that the main immunological and genetic risk factors for RTB is corresponding genotypes of polymorphism T330G gene IL-2, C589T - IL-4, G1082A - IL-10 and C3872T - CRP: the presence in the body of human mutation homozygous or heterozygous genotype; clarified scientific data on cytokines, C-reactive protein in the blood serum and polymorphism T330G gene IL-2, C589T - IL-4, G1082A - IL-10 and C3872T - CRP; found an association between the gene of resistance to rifampicin rpoB Mycobacterium tuberculosis complex and the presence of heterozygous genotype of polymorphism T330G gene IL-2, C589T - IL-4, G1082A - IL-10 and C3872T - CRP. Field of application - medicine.
